  3. List of the Location(s) with Attached Actual Photos of Business Location/Office (Front view and Side view) and Location Map
    (Based on Google App)
  4. Copy of Owner/Authorized Representative’s Two (2) Government Issued Valid IDs
  5. DTI Certificate/SEC Certificate of Registration
  6. Business Permit
  7. BIR COR
  8. Latest Two (2) Years Income Tax Return (ITR)
  9. Latest Two (2) Years Audited Financial Statement (FS) if applicable
  10. Latest General Information Sheet (GIS) if applicable
